Ambulatory Social Worker Case Manager

Job Description :

Pay Range : $38hr - $43hr

Responsibilities :

• Manages an active caseload.
• Provides ambulatory case management services.
• Researches patient clinical history.
• Conducts clinical assessments.
• Provides pre-operative outreach and assessment to student athletes.
• ssists with coordinating medical, social, emotional, insurance, and vocational issues.
• Educates patients and families about advance directives.
• Promotes communication with hospitals and departments.
• Participates in team meetings and huddles.
• Communicates with clinicians, patients, and home care providers.
• Evaluates home care provider network.
• Educates about non-skilled services.
• cts as a liaison between patients and .
• Educates staff about care coordination services.
• Identifies areas for improvement and implements solutions.
• Participates in project development.
• Maintains a resource guide.
• Educates about disease processes and support needs.
• Educates about health insurance processes and benefits.
• Provides supportive counseling.
• Collaborates with Client MA Nurse Case Manager.
• Develops and monitors treatment plans.
• Reports suspected abuse and neglect.
• Participates in case conferences.
• Documents in electronic medical records.
• Provides cross coverage.
• ttends staff meetings.
• Maintains punctuality and attendance.
• Utilizes time productively.
• Participates in staff orientation.
• Identifies learning needs.
