Our 14 community-based homes, including our Northglenn location, are not locked facilities—residents are encouraged to maintain their independence, engage in social interactions, and build friendships.

With local ownership and a dedicated management team, we provide ongoing support to ensure our team members succeed in their roles.

Your Role as a Direct Support Professional (QMAP)

As a Direct Support Professional (QMAP), you will play a key role in supporting our clients’ well-being and independence. Responsibilities include:

Personalized Client Support

• Provide hands-on, individualized care based on client care plans.
• Assist with daily living activities while promoting independence.

Medication Administration

• Administer and track medications per QMAP guidelines.
• Coordinate with pharmacies, verify medication orders, and ensure compliance in collaboration with our nursing team and residents’ PCPs.

Empowerment & Engagement

• Motivate and support residents in achieving personal goals and fostering independence.
• Work closely with our clinical social worker team to provide emotional and mental health support.

Monitoring & Communication

• Accurately document care and support using our electronic medical records system—"If it’s not documented, it didn’t happen!"
• Communicate effectively with the team to ensure seamless care.

Creating a Safe & Supportive Home Environment

• Prepare and serve three nutritious meals daily, along with snacks and hydration options.
• Encourage residents to participate in maintaining a clean and organized living space.
• Conduct room checks and help maintain cleanliness in common areas.

Schedule & Work Details

• Location: Northglenn, CO
• Shifts: Friday – Sunday (12-hour or 24-hour shifts, starting at 7:00 AM)
• Job Types: Full-time, Part-time
• Expected Hours: 12 – 48 per week

Qualifications

✅ QMAP Certification (current)
✅ CPR/First Aid Certification (current)
✅ English Proficiency (multilingual candidates are a plus!)
✅ Compassion & Patience: A caring, client-focused approach
✅ Strong Communication & Teamwork Skills
✅ Experience in caregiving or mental health support is a bonus
