Overview
The ideal candidate will be responsible for providing support and guidance to individuals and families facing various challenges. This role involves assessing resident's needs, and implementing interventions to promote well-being and stability. The Social Worker will work collaboratively with other professionals to ensure comprehensive care for residents.
Responsibilities
- Serve as the resident/family advocate and will coordinate all psychological services involved in patient care from the time of admission until discharge from the healthcare center.
- Ensure the personal and property rights of all residents.
- Be involved in family counseling, mood and behavior management programs, psychotropic medication monitoring and any psychosocial needs of the residents.
- Assist in interpreting various policies and procedures for residents and families and assist with Advance Directive Planning and end of life situations
- Maintain communication with appropriate staff and families regarding resident status and needs and will maintain sufficient records.
- Coordinate with hospital for admissions and discharges to the Health Center.
- Coordinate level of care changes between the levels of care in the Health Center.
- Serve as the Social Work representative on the interdisciplinary team in Care plan meetings.
- Coordinate the monthly care plan schedule.
Skills
- Strong background in social work principles and practices.
- Excellent communication skills, both verbal and written, with the ability to engage diverse populations.
- Bachelor's degree (BA, BS) from a four-year college or university; one-year related experience and/or training; or equivalent combination of education and experience.
