Responsibilities
Oversee the completion of MDS assessments across multiple facilities
- Ensure compliance with federal and state regulations regarding MDS documentation
- Provide training and support to facility staff on MDS processes
- Monitor and analyze MDS data for quality assurance
- Collaborate with interdisciplinary teams to develop and implement care plans
- Facilitate regular meetings to discuss MDS updates and best practices
Respond to inquiries and resolve issues related to MDS documentation
Requirements
Bachelor's or Associate's degree in Nursing
- LPN or RN license in the state of Tennessee
- A minimum of 3 years of experience in skilled nursing facility or healthcare coordination
- Strong knowledge of the MDS process and related regulations
- Excellent communication and interpersonal skills
- Proficiency in data analysis and reporting
- Ability to work independently and manage multiple projects simultaneously
- Certification in MDS (such as MDS Coordinator) is preferred
CMI (Case Mix Index) experience is a Must!
