Weekend On Call LPN/RN (Hospice)

Responsibilities

Duties include but are not limited to:

· Friday 5:00 PM thru Monday 8:30 AM

· Triaging calls after hours on evenings, weekends, and some holidays. Assess needs of the hospice patient, family, and facility staff

· Make PRN skilled nursing visits to address and manage symptoms during on-call hours

· Perform evaluations as needed to determine eligibility

· Meet with families to provide education on hospice services as needed

· Perform initial assessments for admission

· Pronouncement at time of death, post mortem visits

· Observe for patient changes and reports to physician and team

· Provide emotional support to patients, families and facility staff

· Provide education to patient's, families and facility staff regarding end of life care

· Perform nursing procedures as needed as directed by physician orders

Requirements

· Valid Ohio Registered Nursing License

· Licensed driver with an insured vehicle

· Critical thinking, analytical, interpersonal and organizational skills

· Great customer-service minded attitude

· Compassion

· Flexibility

Required Experience

· Nursing: 1 year

· Hospice Experience not required but preferred

· Education: Associates Degree
