Coon Memorial Nursing Home is dedicated to providing compassionate, high-quality care to our residents. We are seeking a Licensed Social Worker (LSW/LMSW/LCSW) to join our team and support the emotional, social, and psychological well-being of our residents and their families.

- Provide case management, advocacy, and counseling services to residents and families.
- Conduct psychosocial assessments and develop individualized care plans.
- Assist with admissions, discharges, and transitions of care, ensuring a smooth process for residents and their families.
- Coordinate with healthcare providers, families, and community resources to address residents’ needs.
- Facilitate support groups, educational sessions, and crisis intervention as needed.
- Ensure compliance with state and federal regulations related to social services in long-term care.
- Collaborate with the interdisciplinary team to enhance residents' quality of life.
Qualifications:
- Current licensure as an LSW, LMSW, or LCSW in the state of Texas.
- Experience in long-term care, skilled nursing, or geriatrics preferred.
- Strong knowledge of Medicare/Medicaid, discharge planning, and resident rights.
- Excellent communication, problem-solving, and advocacy skills.
- Ability to work effectively in a team-oriented environment.
