Responsibilities
Education, Experience and Training:
Bachelor’s degree from an accredited college or university in in public health, social work, education, or related field. One (1) year of work related experience.
Special Requirements/Knowledge, Skills & Abilities
Skilled in the use of standard software applications. Ability to effectively communicate, both verbally and in writing, and establish and maintain effective working relationships with employees, departments and the general public. Must possess a valid Texas Driver’s License, with a good driving record and reliable transportation.
Physical/Environmental Requirements
Standard office environment. Ability to travel to various work site locations.

Responsibilities
- Conducts sexual health interviews and assessments and works with clients to determine HIV/STD prevention needs. Provides status neutral linkage to appropriate services, including linkage to HIV/STD and HCV care, internal/external PrEP linkage, referrals to harm reduction services, and other supportive services as necessary.
- Works with the sexual health clinic and public health follow-up teams to support HIV prevention services, including the provision of PrEP, PEP, and other evidence-based prevention strategies. Develop and implement innovative strategies to improve uptake, adherence, and persistence in PrEP services.
- Promotes HIV/STD prevention services, including regular testing, PrEP, and PEP. Follow-up with clients to support PrEP initiation and persistence (including addressing barriers to medication access, appointments, etc.).
- Assists the sexual health clinic and public health follow-up team to provide the necessary support to initiate and maintain patients on PEP and PrEP as appropriate, including supportive services that can enhance patient-centered risk reduction counseling, insurance, and benefits navigation.
- Collaborates with peer navigators and linkage to care staff as necessary to develop prevention plans for clients at elevated risk of HIV infection.
- Conducts scientific-based prevention education sessions with HIV-positive individuals and individuals at high risk for HIV infection, including individual sessions and workshops. Provides specialized HIV prevention education to HIV positive individuals.
- Develops relationships with community stakeholders to reach HIV positive individuals and individuals at risk for HIV infection in areas with disproportionate HIV rates
- Performs other duties as assigned.
