What are the responsibilities and job description for the Patient Care Navigator (RN/LVN) position at Dallas Nephrology Associates?

Responsibilities of this position will be to identify patients at our Denton North (Corinth) office practice that have been diagnosed as Chronic Kidney Disease stages 4 and 5 to ensure that these patients are receiving appropriate follow-up care that has been designated as the standard of care for patients in these stages of chronic kidney disease. The role of the Patient Navigator will be identify these patients and monitor and coordinate with the attending physician and the other care team members that the appropriate treatment plan is being followed.
Focus will be to ensure that the patient is being prepared for end-stage-renal-disease with emphasis on the following areas:
· CKD Education
· Medical Nutrition Therapy
· Vascular Access Referral and Vascular Access placement if appropriate
· Advance Care Planning if appropriate
· Follow-up Care according to established intervals
· Pre-emptive Transplant referral when appropriate
· Supportive Care referral if appropriate
· Options of Care for end-stage-renal-disease
· Chronic Care Management referral when appropriate
· Discussions regarding dialysis modalities
Qualifications for this position:
- RN or LVN License
- Minimum of one year Nephrology experience
*Pay rate is based upon candidate's level of nursing experience*

Pay: $50,000.00 - $80,000.00 per year
