Job Summary

The Annual Wellness Visit nurse works in collaboration with the primary care provider and all health care team members. Primary responsibilities include the identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and the execution of guided processes to complete the AWV. Assesses healthcare needs that require additional follow-up by the primary care provider and assists patients in scheduling appropriate follow-up. Knowledgeable of community resources and assists patients in connecting with those resources. Serves as a patient advocate and assists in identifying and improving service delivery. This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations.

Responsibilities & Duties

- Provides annual Medicare wellness visits for DFP patients.

- Identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV.

- Follow AWV protocols and standing orders including completion of preventive services that may result from AWV.

- Will work collaboratively with the healthcare team and refer patients for care coordination and health coaching when appropriate.

- Make recommendations for members’ follow-up with their primary care physician.

- Acts as an advocate for patient and helps them to obtain their maximum level of health. Provides education to patients about their specific conditions and needs.

- Address open quality gaps with patients and works collaboratively with patient towards closure of those gaps.

- Maintains EHR documentation and data. Completes all documentation in a timely manner.

- Participates in regular team and organizational meetings. Participates on committees as applicable. Assists in the orientation of new personnel. Precepts and mentors other staff members. Promotes a collaborative atmosphere across the organization.

- Provides educational opportunities and is a resource person for providers and staff.

- Meets with practice manager regularly to provide patient and staff updates, identify issues, and develop strategies for resolution.

Requirements

- Current Georgia Registered Nurse license

- CPR certification

- Two to four years of nursing in primary care, community health nursing, critical care, or home health preferred.

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
