Licensed Voc. Nurse - Tele (Nights)

Job Summary
The Licensed Vocational Nurse (LVN) is a member of the nursing team who provides technical nursing care to a diverse group of patients in the Telemetry (Tele) unit, under the supervision of a Registered Nurse (RN). The LVN monitors cardiac rhythms, assists in patient teaching assignments, and may assume leadership responsibilities by supervising unlicensed assistive personnel (e.g., CNAs).

Essential Functions

1. LVN – Organizes and directs shift responsibilities and duties within the Telemetry unit.
2. Participate in the assessment, planning, implementation, and evaluation of patient care in collaboration with the RN, focusing on cardiac monitoring and hemodynamic stability.
3. LVN – Make determinations whether a physician should be called (prepare for the call using the SBAR (Situation, Background, Assessment, Recommendations) tool), especially regarding changes in cardiac rhythms or vital signs, and coordinate with the RN as needed.
4. Accurately prepare and administer medications and intramuscular/subcutaneous injections according to hospital policy and scope of practice.
5. May initiate and maintain IV infusions. May infuse vitamins, electrolytes, blood, or blood products after successful completion of an accredited IV therapy course. May instill minute amount of Heparin for the sole purpose of maintaining lock patency.
6. Provide direct care to patients following hospital policies and procedures. Acts as a role model for the staff that she/he works with.
7. Perform treatments (e.g., wound care, dressing changes) and assist with procedures as directed. If properly trained, may perform glucose monitoring utilizing a glucose analyzer.
8. Recognize, report, and record any change in condition or behavior of the patient; monitor telemetry readings closely and alert the RN/physician of abnormalities.
9. Assist with orientation of new employees and share expertise with peers.
10. Make regular rounds on assigned patients to monitor vital signs, pain levels, and overall comfort, with particular attention to cardiac monitoring.
11. LVN – Oversees and communicates with CNAs to ensure that they are performing their duties correctly by periodically verifying the quality and status of their work.
12. Accurately communicate in writing using the appropriate charting method (EMR), including patient care summaries and proper transcription of medical orders.
13. Establish therapeutic rapport with patients, families, and visitors, while respecting patients’ rights to privacy and confidentiality of information.
14. Communicate pertinent information regarding patients at shift report, ensuring continuity of care.
15. LVN – Acts as a liaison between the CNAs and the Charge Nurse/Department Leadership.
16. Recognize and respond to priorities in the patient care process and adapt to changes in workload.
17. Provide proper notification for absence and tardiness; be punctual at the start of each shift.
18. Observe hospital dress code and wear identification badge.
19. Maintain a clean and safe environment for patients and other nursing staff.
20. Demonstrate understanding of emergency situations and response plans (e.g., Code Blue, Rapid Response) and be prepared to assist with cardiac emergencies.
21. Assist in maintaining equipment assigned to patient care, particularly telemetry monitors; report any malfunctioning equipment to appropriate personnel.
22. Attend mandatory meetings and in-services.
23. Complete and document narcotic count as required.
24. Responsible for observing safety precautions.
25. Follow all general and departmental safety, security, and health procedures and policies. Utilize all recommended safe work practices.
26. Perform other duties as assigned.

Qualifications

• High school graduate
• Graduation from an accredited Licensed Vocational Nursing program
• Current license from the California State Board of Vocational Nursing
• Current IV certification
• Current Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) may be required based on facility policy
• Experience in Telemetry or cardiac monitoring preferred
