OUTREACH/HIV PREVENTION SPECIALIST:
QUALIFICATIONS:
Bachelor’s degree from an accredited college in social work; counseling; nursing; health education or related field preferred.
One year of experience working in HIV prevention or care, medical care systems, with diverse populations, and or underserved communities required.
Demonstrated ability to provide services with cultural competence.
Must possess excellent organizational, interpersonal and communication skills to interact, develop, and sustain sound working relationships with patients, families, multidisciplinary professionals, other departments within DFCC and outside agencies.
· Bi-lingual skills in English and Spanish are strongly preferred.
· Community based work required.
· Experience working with an EMR.
POSITION SUMMARY:
Working with the Firehouse Health care team, including the Director of Special Populations, medical providers, nurses, social work, and other support staff, the Prevention Specialist is responsible for the coordination and support of PEP/PrEP patients through the screening, assessment, care planning, and treatment processes.
ESSENTIAL FUNCTIONS:
Provides confidential assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services.
Provide ongoing follow up to ensure PrEP adherence. Schedule initial and follow up visits and call all scheduled PEP/PrEP patients 24 to 48 hours before a schedule visit.
Reduce barriers to obtaining clinical PEP/PrEP care/services. Assess benefits and insurance, and if needed, provide assistance to access insurance programs (Adap/Gilead).
Provide PEP/PrEP disease prevention and PEP/PrEP adherence education and counseling.
Collaborate with all individuals involved in PEP/PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
Provide service linkage to address other psychosocial needs as appropriate; make referrals for more complex psychosocial intervention as indicated.
Implement PrEP/PEP awareness campaigns and social events focused on enrolling new patients.
Attend and participate in HIV prevention meetings such as the Queens Knows HIV provider network.
Ability to fully utilize an electronic record system to perform essential job functions, which include but are not limited to (1) documentation in EHR, (2) transmission of EHI, (3) retrieval of EHI.
Verbalizes knowledge of and demonstrates sensitivity to the patient’s bill of rights, maintains patient confidentiality at all times, and adheres to HIPAA Privacy and Security Rules.
Documents all patient interventions in the EHR in accordance with established DFCC policy/procedure and demonstrates compliance to all organizational policies.
Meets all organizational requirements for mandatory education at time of employment and annually, thereafter.
Participates in meetings, committee activities, and the DFCC QI program as assigned.
Performs within scope of practice, demonstrates awareness of legal issues, and utilizes appropriate judgment and actions to reduce potential adverse consequences for patients, self, and the organization.
Maintains open communication with all staff to promote a collaborative and collegial relationship.
Demonstrates a commitment to professional growth by attending educational seminars and keeping current with health care trends.
Demonstrates respect for the cultural and religious diversities and needs of patients, families and care providers.

Pay: $45,000.00 - $55,000.00 per year
