Position Summary
The Health Navigator will provide case management to persons identified as hepatitis C positive. This position will assess eligible patients for needs, identify barriers to hepatitis treatment, and then connect patients with resources to reduce or eliminate those barriers. The Health Navigator will work closely with other departments at The Damien Center to help their patients reduce barriers and receive treatment.

Duties And Responsibilities

- Link identified positive patients to medical care and treatment.
- Assist patients with applying for medical insurance and patient assistance programs as needed.
- Provide patients with education about Hepatitis C, and Harm Reduction and PrEP.
- Test clients for hepatitis C as needed.
- Develop and maintain a strong working relationship with Damien Clinic and external medical clinics for the purpose of referring clients to medical care and treatment.
- Build working relationships with local providers and other community partners who work with the focus population.
- Ensure patient engagement and retention in care by sending appointment reminders, following up after appointments, and frequently checking in with patients about their needs.
- Refer clients to additional services such as STI treatment, substance use treatment, and supportive social services.
- Be able to travel to and work out of multiple locations and workspaces.
- Document all patient encounters and other required data in the appropriate databases.
- Participate in testing and outreach events as needed.
- Attend regularly scheduled calls with the Monthly Connect to Cure Programmatic Manager.
- Regulate and follow-up with programmatic resources for clients on a monthly and quaternary basis.
- Perform special assignments and other duties, on an as-needed basis.
- Bachelor’s degree preferred
- Experience in public health, social work, insurance enrollment, counseling or related field
- Work or lived experience may substitute for education requirements on a case-by-case basis.
