The Dane County Human Services Department, Behavioral Health division, has a full-time (1.0 FTE) vacancy for a Case Manager (Behavioral Health - Comprehensive Community Services) position in Madison, WI. This current opening is in the Comprehensive Community Services unit.

DEFINITION

Case Managers work closely with children, adults, families, guardians, service providers and natural supports. Under the direction of the assigned Unit Supervisor, this position works with individuals in the community to assess their psychosocial needs, determine eligibility for specific services, and connect them with identified programs.

Examples of Duties

Case Managers may work in one (1) of the following areas:

Behavioral Health Resource Center (BHRC): Provide timely accurate and informed resource and referral information to Dane County residents seeking access to behavioral health services. Work closely with county and community partners to identify and advocate for individuals who would benefit from assistance in accessing services. Advocate for those who experience barriers to accessing behavioral health treatment. Communicate with providers, insurers, and other third-party professionals to assist in service delivery throughout systems. Complete referrals and coordinate warm handoffs to behavioral health services and systems including crisis services. Provide active and back up direct services and consultation via phone, email, and in office/community for BHRC consumers and concerned others. Provide behavioral health consultation as requested to county and community-based professionals. Manage an active caseload in collaboration with other team members. Document all calls, contacts, consultations, referrals and activities in database or record keeping system. Provide person centered follow-up services after making a referral to verify referral completion. Assist in decreasing barriers to accessing and engaging with ongoing treatment relationships with behavioral health providers. Consult and team with other Dane County Department of Human Services (DCDHS) programs (For example: Child Protection Services, Case Management Programs, Residential Substance Use Services, Aging and Disability Resource Center, etc.). Consult and team with community-based programs and services to ensure access to services, service prioritization, and authorizations. Collect and document data related to access to services, provider preferences, and barriers to services. Participate in county and community-based meetings and consultation groups aimed to coordinate/increase access to behavioral health services in Dane County. Assist with development and maintenance of BHRC website, behavioral health resource database, and other targeted resource materials. Perform other duties as assigned.

Comprehensive Community Services (CCS): Under the direction of a Social Work Supervisor, incumbents determine program eligibility for persons with mental illnesses and/or substance use disorders seeking Medicaid CCS. Meet with persons who have a mental health and/or substance use diagnosis to educate them about the features of the CCS program and its Recovery model. Ascertain voluntary interest in CCS participation. Gather assessment information via direct client interview and relevant collateral contacts. Complete Department of Health Services (DHS) functional screen to determine functional eligibility. Assist consumer with obtaining physician's prescription as necessary. Ensure that all required documentation is complete and filed in a timely manner. Assist consumers with selecting a service facilitation agency and facilitate a warm transfer to that agency for ongoing services. Assess need for immediate services and make arrangements if necessary. For those determined ineligible or not interested in CCS, provide information and assistance in accessing alternative resources. Perform annual reassessments of eligibility with DHS functional screen. Provide community education on the CCS program. Outreach to other service access points. Perform other duties as assigned.

EMPLOYMENT STANDARDS

Education, Training and Experience:

Requires a Bachelor's Degree in a human services field (social work, psychology, sociology, nursing, counseling, etc.) and the equivalent of one (1) year of full-time experience working in a treatment setting with persons with mental illness and/or substance use disorders. Experience is defined as working directly with individuals identified as part of the target population in a setting where the primary function is assessment, treatment, intervention, or observation of mental health and/or substance use conditions.

All employees hired with a Master’s degree and six (6) months of relevant experience or a Bachelor’s degree and two and a half (2.5) years of relevant experience will be hired as a Case Manager II, range 20. Those who do not meet these criteria upon hire will be hired as Case Manager I and will advance to Case Manager II after obtaining one of these criteria. Applicants must also have the equivalent of one (1) year of full-time experience working in a treatment setting with persons with mental illness and/or substance use disorders. Experience is defined as working directly with individuals identified as part of the target population in a setting where the primary function is assessment, treatment, intervention, or observation of mental health and/or substance use conditions.

Certifications/Licenses:

Candidates may be required to pass State of Wisconsin Mental Health Functional Screen test within 90 days of employment and maintain eligibility to complete the screen. Requires a valid driver’s license, access to reliable personal transportation and a good driving record.

Special Requirements:

BHRC positions may be required to occasionally work on evenings or weekends.

Background Check Statement: Some positions may require a criminal background check which can include fingerprinting due to the nature of the job’s responsibilities. Wisconsin’s Fair Employment Law, s. 111.31 – 111.395, Wis. Stats., prohibits discrimination because of an arrest or conviction record. However, Dane County may disqualify an applicant if the position’s responsibilities are substantially related to the applicant’s criminal history (e.g., the nature of the crime and its relationship to the position, whether hiring, transferring or promoting an applicant would pose an unreasonable risk to the business, its employees, customers and vendors, etc.) Management reserves the right to make employment contingent upon successful completion of the background check.

For Bilingual Positions:

1) Provide services in both English and the required foreign language (e.g., Spanish, Arabic, Hmong, etc.).

2) Applicants being considered for bilingual positions will be tested during the recruitment process prior to employment. This will primarily consist of passing an oral language proficiency assessment from English to foreign language and foreign language to English, but may require additional language testing, such as written translation, if it is an essential function of the job.

* If applicable, individuals may confirm with the Employee Relations Division whether results of bilingual language certifications/ examinations they currently hold qualify them for the position’s bilingual designation.

Physical and Environmental Work Requirements:

Office environment. Ability to see and hear within normal ranges with or without adaptive devices. Ability to use finger, wrist, hand and arm motions. Ability to sit and stand.

Knowledge, Skills and Abilities:

Basic understanding of human development and behavior. Knowledge of current human service philosophies and approaches such as self-determination, person-centered planning, community inclusion, trauma informed care and asset-based community development. Knowledge of and experience applying case management methods, techniques and policies,. Knowledge of investigation procedures and applying motivational interviewing techniques. Knowledge of computer based applications including Microsoft Word, Excel, and other related software. Ability to use a computer to generate reports, complete forms, and document case activity. Ability to prepare and maintain complex records and reports. Ability to develop positive relationships with people, understand their concerns and act to address them in a timely, respectful and effective manner. Ability to quickly acquire and apply knowledge of laws, regulations, policies and procedures relating to the target population. Knowledge of and ability to access community resources. Ability to prioritize, calmly manage and resolve urgent or crisis situations. Capacity to organize large amounts of information and manage complex situations to achieve the most effective individual and programmatic outcomes. Ability to maintain an excellent standard of customer service driven by a compassionate, empathetic, and diplomatic approach. Excellent personal, written and verbal communication skills. Ability to follow oral and written instructions, autonomously meet deadlines, participate in and appropriately use supervision. Ability to participate effectively as a team-member. Ability to engage and work with people from diverse ethnic and cultural backgrounds with understanding and respect for the individuals and their differences.

Pay: $33.86 - $39.52 per hour
