The Dane County Department of Human Services has a current vacancy for a .5 FTE Project Lead Social Worker in DAS - DSCTU (Dementia Support & Care Transition Unit).  NOTE: This is a project position, contingent on available funding, currently through 12/31/2025.

DEFINITION 

Under the direction of the Manager of APS, and collaboration with the Dementia Stabilization Care & Treatment Unit Project Manager (DSCTU), the purpose of this Lead SW project position would be to develop all policy and procedure documents and training materials consistent with regulations for newly licensed unit in a local nursing home. The Lead SW will also work closely with the Dementia Stabilization and Care Transition Unit (DSCTU) Project Manager. The position is dedicated solely to the planning for the DSCTU.  They are responsible for drafting and completing final documents upon approval of all admission and discharge protocols, policies and practices/procedures and required forms for the opening of said unit upon completion of the planning phase.   This position will require advanced knowledge of nursing home procedures related to the admission, stay, and discharge of residents as required by regulations using a trauma informed lens due to the nature of the admission during crisis and dysregulation.  It will also require an advanced knowledge of dementia and best practice strategies to support a person with dementia in their moments of crisis and dysregulation.  

 

EXAMPLES OF DUTIES 

Working with the DSCTU Project Manager the Lead Social Worker will be responsible for the research and development of all social work related policies and procedures for the DSCTU.   These will include admission procedures, grievance procedures, social / Life Story assessments, care plans, discharge planning, notice of discharge, discharge summaries, language access, etc.   In addition, they will assist in researching and creating dementia informed training protocols for care staff in the DSCTU to ensure staff are competent and confident in their ability to support and care for persons with dementia during their times of dysregulation.

Bachelor’s or Master's Degree from an accredited college or university with a major in Social Work, or a related human services field. Candidate must have at least two (2) years of professional experience in the development and application of policies and procedures to meet social work related compliance standards of Wisconsin Statute Chapter 50 (Uniform License – Care and Service Residential Facilities) State of Wisconsin regulations (WI Administrative Code DHS 32),  and Federal nursing home regulations.  They will also have at least two (2) years of experience supporting persons with dementia, their families, caregivers and dementia related programs.


 Certifications/Licenses: 

All individuals must possess a degree and permanent Social Work Certification from the WI DSPS prior to their first day of employment. If any applicant does not possess a permanent certification on the first day of employment the job offer will be rescinded and they will be terminated

 

Special Requirements: 

Must have a valid driver’s license, a good driving record and access to reliable personal transportation, or equivalent. 

 

Background Check Statement

For Bilingual Positions: 

  1. Provide services in both English and the required foreign language (e.g., Spanish, Arabic, Hmong, etc.). 
  2. Applicants being considered for bilingual positions will be tested during the recruitment process prior to employment. This will primarily consist of passing an oral language proficiency assessment from English to foreign language and foreign language to English, but may require additional language testing, such as written translation, if it is an essential function of the job. 

*  If applicable, individuals may confirm with the Employee Relations Division whether results of bilingual language certifications/ examinations they currently hold qualify them for the position’s bilingual designation.


Physical and Environmental Work Requirements: 

Requires sitting and standing; work inside and outside of an office environment which includes home visits and meetings with service providers. Ability to sit or stand to operate a personal computer with or without assistance. Ability to travel from office to other locations. Ability to see and hear within normal ranges with or without adaptive devices.

Excellent interpersonal, verbal and written communication skills. Ability to synthesize information for documentation in required formats. Familiarity with Wisconsin Nursing Home Codes/Regulations and protective services standards for adults. Knowledge of the Dementia informed best practices. Knowledge of community resources and how to use them effectively. Knowledge of family and community systems. Proven ability to convene and lead project stakeholders to achieve project results.  Ability to relate to persons from a variety of ethnic and racial backgrounds in an unprejudiced and understanding manner. Ability to recognize symptoms of cognitive disabilities. Ability to prepare and maintain complex records, documents and reports. Ability to participate effectively as a member of a team. Ability to plan and organize work to effectively achieve project objectives. Ability to understand and follow oral and/or written instructions. Ability to use a computer to generate reports, complete forms, and document activity. Demonstrated ability to adapt to change and be flexible regarding learning new skills and new practice methods for project related tasks
