Job Summary:
The Women’s Health Patient Navigator (WHPN) is responsible for providing health care systems navigation for women who seek breast and cervical health services. The WHPN will guide women through the cancer “continuum of care” to be screened for breast and cervical cancer, receive any necessary diagnostic care, get treatment if cancer is diagnosed and receive follow-up care after treatment. The WHPN will work to ensure timely entry into the continuum, timely result notification of screening and diagnostic care, assess for gaps in service availability, and identify barriers faced and what can be done to address those gaps and barriers. The WHPN will also provide patients and families with education and information regarding the continuum of care process. The WHPN serves as an important facilitator among the patient, family, or caregiver, and health team.

Essential Duties and Responsibilities:
Administrative Functions:
- Process and manage the life cycle of breast and cervical related referrals (screening and/or diagnostic) according to standards and to level of urgency.
- Appropriately co-manage Pap test results with Nursing by running the Pap Reconciliation Reports and weekly huddles.
- Track breast and cervical related referral reports to ensure results are obtained in a timely manner to move through the continuum of care.
- Data entry of all breast and cervical cases in the i2i referral module, pap tracking, and mammogram tracking.
- Send breast and cervical screening recalls, result notification, and when unable to reach by phone letters to patients according to letter grid.
- Instruct patients on breast and/ or cervical procedural preparation as applicable.
- Schedule and reschedule patient appointments for continuity of care.
- Facilitate communication and coordination between members of the health care team, involving the patient in the decision-making process in order to minimize delays through the continuum of care.
- Conduct intakes and assessments to assess barriers for adherence to diagnostic work-up and/or cancer treatment.
- Develop and document a navigation plan collaboratively with the patient, family or caregiver, and provider based on assessment findings.
- Refer patients who need diagnostic work-up or diagnosed with cancer to community agencies for services.
- Ensure patients diagnosed with cancer enter and receive treatment in a timely manner through timely case processing.
- Maintain ongoing and appropriate communication with patients throughout the continuum of care.
- Work with care team to maximize healthcare responses, quality, and cost effective outcomes.
- Document all interventions to include, date, and time based on protocols in EHR. Document accurately, appropriately and efficiently all interactions with patients in EHR.
- Complete Every Woman Counts cases in DETEC within standards. Follow Case-Management billing process for eligible cases, and document in Billing Report in a timely manner.
- Advocate and educate with compassion and empathy for the patient to ensure patient has the knowledge to care for his/her condition, is empowered to be responsible for participating in the plan of care to facilitate positive outcomes.
- Seek assistance of Nurse Manager/LVN with complex abnormal or cancer cases to avoid clinical consequences of delay.
- Seek assistance of Clinic Manager/ WHPN Lead, or Director of Women’s Health when unable to completed breast/cervical cases to avoid backlog.
- Make contact with all relevant healthcare facilities that might also provide diagnostic, treatment or social services to patients.
- Conduct recalls for women due for breast and or cervical screening services.
- Comply with cervical and breast cancer screening guidelines, result notification, management of abnormal, and referral management protocols.
- Assist with mobile mammography events at the clinic.
- Handle all incoming calls for the department in a courteous manner, providing callers an answer to their questions as best as possible.
- Provide quality care to the patients within the scope of practice outlined by state or federal law.

Qualifications
Qualifications:
Minimum Qualifications (Education and Experience):
- Diploma from an approved High School or GED Equivalent.
- Two years of breast/cervical case-management/navigation or two years of case-management/navigation in another field.
- Knowledge of community services, social service support agencies, and resources for cancer patients.
- One year working in a woman’s health or community health setting.
- Knowledge of breast and cervical health education and services.
- Experience with providing patient education.
- Knowledge of available public funded programs including Every Woman Counts, FPACT, Breast Cervical Cancer Treatment Program (BCCTP) and others as determined appropriate.
Knowledge, Skills and Abilities:

- Medical terminology and computer skills.
- Bilingual in Spanish/English preferred.
Additional Requirements: (Licenses/Certifications)
- All staff that is presently working within a clinic is required to have a Current and Valid CPR and First Aide certification on file with the Human Resources Department. This is for clinical and administrative staff. This new license requirement is mandatory for continued employment, CPR and First Aide certification must be obtained through the American Heart Association. Internet certificates do not comply and for this reason are not valid.
