DARBYSHEALTHCARE is a community-based homecare agency dedicated to delivering quality, compassionate, and personalized care to patients in the comfort of their homes. We are expanding our clinical team and looking for experienced and motivated Family Nurse Practitioners (FNPs) to join our mission of improving access to primary and preventative care for homebound individuals.
Position Summary:
The Family Nurse Practitioner (FNP) will provide comprehensive medical care in home-based settings. This role includes performing in-home physical examinations, diagnosing and treating common illnesses, prescribing medications, managing chronic conditions, and coordinating patient care with our interdisciplinary team.
Key Responsibilities:
- Perform comprehensive physical assessments during in-home visits.
- Diagnose and manage acute and chronic conditions such as hypertension, diabetes, COPD, etc.
- Prescribe medications and develop individualized treatment plans.
- Order and interpret diagnostic and laboratory tests as needed.
- Provide vaccinations and immunizations according to clinical guidelines.
- Deliver patient and family education on disease prevention, medication management, and lifestyle modification.
- Monitor patient progress through regular follow-up visits.
- Coordinate care with physicians, specialists, home health aides, and other healthcare professionals.
- Document all visits and patient interactions thoroughly and accurately in the EMR system.
- Provide counseling and support to patients and families to manage medical conditions.
- Participate in care team meetings and contribute to ongoing quality improvement efforts.
Qualifications:
- Licensed and certified Family Nurse Practitioner in the State of [Insert State].
- Master’s Degree in Nursing (MSN) from an accredited program.
- Active and unrestricted NP license and prescriptive authority.
