Under limited, general supervision, the Residential Programmer will provide direction and guidance of staff to ensure program monitoring for consumers in the CTH II Homes. Services and supports provided to consumers will be consistent with the agency’s vision/mission.
Job Duties:
Will monitor all goals and objectives for each residential consumer living in a CTH II home.
Success Criteria:
· Will write or revise goals and objectives to be implemented by staff.
· Will assure that goals and objectives are clear and easy to implement by the staff.
· Will assure that each goal is written on the proper forms, monthly summary sheet, data collection sheet, and training steps form and each form is placed in the consumer data book.
· Will assure that all objectives are monitored and tallied each month, a monthly percentage has been determined and the percentage is documented on the monthly summary sheet.
· Will provide CTH II staff with training materials needed to implement objectives.
· Each month will determine weather consumers have received a met progress, satisfactory progress, or unsatisfactory progress.
· Will determine if a goal need to be discontinued, revised or continued.
· Will develop a procedure to assure that at least 10 days of data is collected on goals every month.
· Will assure that goal books have data collection sheets in them so that staff can properly document the data collected.
· Will assure that training steps have been developed for each goal, so that staff has instructions on how goals should be run.
Will complete comprehensive function assessments.
Success Criteria:
· Will assure that an annual assessment is completed on each consumer by the house manager and placed on goals that are based on the annual assessment.
· Based on the annual assessments, will determine if current goals and objectives need to continue, be revised, or discontinued. Will also determine if a new goal needs to start.
Will complete a quarterly report and document on the report all interventions made during the quarter.
Success Criteria:
· Will assure that a Quarterly Review report has been completed on each consumer.
· Will assure that the Quarterly Review report addresses the following, consumer’s health and medical visits, psychotropic medications, behavior (BSP), call teams or amendments, social/recreation, exercise, financial summary, and goals/ objective, training in rights, making personal choices and abuse and neglect training.
· Will assure that copies of Quarterly Review have been sent to the consumer’s Service Coordinator and filed in each consumer records.
Will provide training to staff in the implementation of objectives and BSP’s.
Success Criteria:
· Will become familiar with consumers BSP’s.
· Will meet with the behavioral specialist for training on BSP’s.
· Will train staff on all BSP’s.
· Will provide training to staff on all new or revise goals and objective.
· Will conduct in-service for staff whenever there is a concern or changes related to the collection of data.
Will perform other related duties.
Success Criteria:
· Oversee and ensure a safe and healthy environment for consumers in the home.
· Ensure that a yearly disaster drills is conducted.
· Will complete functional assessments for consumers on an annual basis, upon admission, and as needed.
· Assist consumers by arranging for transportation.
· Ensure that medical supplies, medication, household supplies are sufficient and current.
· Will carry a cell phone during working hours and other times as needed, depending upon needs of homes/service.
· Management of consumer Funds and complete monthly and quarterly financial reports.
· Will ensure that consumer’s behavior support plans are being monitored correctly and are trained on goals and objectives.
· Ensure inventory in excess of $50.00 is documented quarterly or when items are purchased.
· Will assure that quarterly inventory for items $50.00 and less is completed.
· Oversee the Participation of Call Team meetings and Residential Plans.
· Know and verify all CTH standards are met.
· Will ensure that service provision log is completed daily and turned in each week.
· Will assure that consumer’s records (20 tabs) are kept organized in a consistent manner throughout the CTH II program.
· Will assist with monthly review check sheet to assure that consumer’s annual medical appointments, annual exams, and annual consents are current.
As a member of the DCDSNB Executive Team assists with agency duties. Meets with the team on a regular basis to assist in the coordination and oversight of the activities of the agency. Provides agency training as a member of the training cadre. Performs administrative on-call on a rotating basis. Assists with other duties as assigned by the Residential Director.
A. Provides professional opinion about issues, problems and concerns.
B. Assists in finding solutions to agency problems.
C. Is cooperative and works with others on the team in a productive manner.
D. Maintains required certifications for training responsibilities.
E. Provides documentation as required by the Director of Training.
F. All administrative on-calls are handled in a prompt and professional manner and documented in the log.
G. Shows willingness to assume minor duties for the benefit of the agency.
H. Successful completion of assigned tasks.
Minimum Requirements: High school diploma and five years’ experience working with people with disabilities. Will have proficiency in the use of computers for word processing, maintenance of data files and spreadsheets. Ability to prioritize tasks and handle multiple projects at one time. Must possess a valid S.C. driver’s license.
Physical Requirements: Reading, writing, driving, standing, walking, sitting, lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, crawling, reaching, handling, speaking, hearing, seeing, depth perception and color vision.
