Overview
The Community Health Worker (CHW) plays a vital role in connecting families and communities to essential early childhood services, reproductive and sexual health care, mental health support, men's support services, parenting resources, and overall health care. This position aims to bridge the health equity gap by supporting individuals and families through education, advocacy, and resource navigation.
Key Responsibilities
- Outreach and Engagement:
- Conduct community outreach to raise awareness about available services and resources.
- Build trusting relationships with community members to encourage participation in programs.
- Resource Navigation:
- Assist families in identifying and accessing appropriate health and social services.
- Maintain an up-to-date directory of local resources and service providers.
- Education and Advocacy:
- Provide education on health equity, health literacy, and available community resources.
- Advocate for the needs of individuals and families within the community.
- Support and Follow-Up:
- Offer ongoing support and check-ins with families to ensure they are receiving necessary services.
- Facilitate group workshops and informational sessions on relevant topics.
- Collaboration:
- Work closely with federally qualified healthcare centers, local health departments, educational institutions, and other community organizations to enhance service delivery.
- Participate in collaborative meetings and networking opportunities.
- Data Collection and Reporting:
- Track and report data on services provided and community engagement outcomes.
- Gather feedback from families to assess program effectiveness and identify areas for improvement.
Qualifications
- Education:
- Bachelor's degree in health services, social work, or related field preferred. Minimum requirement: high school diploma.
- Must be a Certified Community Health Worker in accordance with the South Carolina state-approved certification process or achieve certification within 90 Days of acceptance of the position. Training will be available.
- Experience:
- Previous experience in community outreach, health education, or a related field is preferred.
- Familiarity with local health services and resources.
- Skills/Knowledge:
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Cultural competency and sensitivity to diverse populations.
- Basic knowledge of mental health first aid is a plus.
- Must have an in-depth or comprehensive knowledge of the community served.
