At Dartmouth Health, we are seeking a dedicated Annual Wellness Visit Nurse to join our team. This role is responsible for coordinating and implementing primary care annual wellness exams, following established guidelines and documentation requirements.
The ideal candidate will have five years of clinical experience, with at least one year in a medical office setting. A valid NY and VT Nursing license is required, as well as BLS certification.
This position requires strong communication and organizational skills, with the ability to work independently and as part of a team. The Annual Wellness Visit Nurse will also be responsible for outreach and support development of annual wellness exam protocols and standing orders.
Required Skills and Qualifications:
- Graduation from an accredited school of nursing
- Five years of clinical experience
