Contract Duration: 3 months
Shift: 8:00 AM - 4:30 PM (40-hour guarantee, 8-hour days)

Job Description:
Berkshire Medical Center and North Adams Hospital are seeking a Licensed Clinical Social Worker (LCSW) to join our team as a Case Manager. This position plays a critical role in managing the healthcare needs, quality outcomes, and cost-effectiveness of patients across the continuum of care. The Case Manager will utilize a multidisciplinary team approach to coordinate and monitor services, resources, and patient care.
The role is responsible for reviewing patient needs at admission, throughout the hospital stay, and in some instances prior to admission, ensuring that the level of care and utilization of resources is appropriate. The position will float between Berkshire Medical Center and North Adams Hospital.
Key Responsibilities:
- Manage the healthcare needs and services for patients, ensuring quality and cost-effective outcomes.
- Facilitate and coordinate patient care across multiple disciplines and service lines.
- Review patients at admission, during hospitalization, and upon discharge to ensure appropriate levels of care.
- Utilize CMS guidelines, appeal rights, and DPH regulations for care management.
- Conduct risk assessments related to quality, financial, and payer benefits for each patient.
- Utilize evidence-based criteria tools for utilization management (levels of care knowledge).
- Collaborate with healthcare providers, insurance organizations, patients, and families to develop and implement care plans.
- Provide expertise in discharge planning and facilitate transitions of care to appropriate post-acute settings.
- Document and report findings, including summaries for management, legal, and regulatory agencies when needed.
- Maintain confidentiality and ensure compliance with all regulatory and accreditation standards, including those of Joint Commission, DPH, and HealthGrades.
Qualifications:
- Master's in Social Work (MSW) required.
- Licensed Clinical Social Worker (LCSW) required; LICSW preferred.
- 3 years of clinical experience in acute care settings (internal medicine, surgery, orthopedics, cardiology, oncology, psychiatry, etc.).
- 3 years of clinical experience in rehab settings, home health, or long-term care is a plus.
- Strong understanding of utilization management, discharge planning, and CMS guidelines.
- Knowledge of Kepro QIO and appeals processes.
- Familiarity with regulatory organizations such as Joint Commission, DPH, and HealthGrades.
- BLS (AHA) certification required.
- Massachusetts State License required.

- Proven collaboration and negotiation skills with providers, patients, families, insurance organizations, and government agencies.
- Ability to manage multiple tasks and applications simultaneously in a fast-paced environment.
- Proven excellent interpersonal and communication skills.
- Ability to perform comprehensive assessments and prepare detailed reports.
- Ability to work independently and effectively manage a caseload.
- Knowledge of Medicare, Medicaid, and third-party commercial payer systems.
Additional Requirements:

Compensation & Benefits:
- Competitive hourly rate (based on experience).
- OT and holiday multipliers at 1.25%.
- Non-billable Orientation: 8 hours.
