Social Worker
Work Hours:9am-5pm or 10am-6pm
lunch period: 1/2 hour Unpaid
Assignment location: Preakness Healthcare 305 Oldham Road Wayne, NJ 07470
Job description:-
- Perform resident assessments.
- Provide resident-centered care as a member of the interdisciplinary care team.
- Plan for resident discharges and maintain required documentation.
- Provide counseling and guidance to residents and their families.
- Provides information on outreach services offered to families.
- Ability to establish and maintain cooperative relationships with clients and staff.
Long-term care experience Social Worker in a nursing home facility. Must be able to assess/document in MDS.
Mandatory skills/certification(s) Requirement:- LSW, MSW
Job Type: Contract
Pay: $44.00 - $44.64 per hour
Schedule:
- 8 hour shift
- Monday to Friday
Work Location: In person
Salary : $44 - $45
