JOB DESCRIPTION SUMMARY
Responsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Responsible for managing all aspects of maintaining and protecting organization clinical records including the management of the clinical records team.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
2. Ensures compliance with all state and federal referral/intake regulatory requirements.
3. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation, and coordination of care with multiple service providers.
6. Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
7. Negotiates service pricing with insurance Case Managers and other payers within established financial and credit parameters.
8. Assists the Executive Director/Administrator in the preparation of an annual budget for the intake department and monitors allocation of resources according to budgetary limitations.
9. Gathers, collates, and reports referral statistics including key customer referral trends.
10. Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
11. Maintains comprehensive working knowledge of Emunah Hospice's contractual relationships and ensures that patients are admitted according to contract provisions.
12. Participates in quality assessment performance improvement teams and activities.
13. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Emunah Hospice.
14. Manages all daily clinical records functions including establishing and implementing clinical records policies.
15. Ensures clinical record systems are maintained in compliance with state and federal regulations.
16. Maintains comprehensive working knowledge of state and federal documentation regulations and serves as a resource for appropriate organization personnel.
17. Protects all clinical records through the establishment and implementation of control procedures for all open and closed records.
18. Protects the confidentiality of patient and organization information through effective controls and direct supervision of clinical records.
19. Forwards copies of clinical records to authorized users according to policy.
20. Completes an administrative record audit following patient discharge and forwards the record to the Clinical Supervisor for clinical audit.
21. Monitors aged unsigned physician orders and submits second and third requests for signed orders to overdue physicians, calls or visits the offices of seriously overdue physicians.
22. Supervises the use of the clinical records information system and maintains a comprehensive working knowledge of the system including upgrades and enhancements.
23. Ensures accurate and prompt filing and retrieval of clinical records.

POSITION QUALIFICATIONS
1. Licensed practical nurse with current licensure to practice professional nursing in the state.
2. At least one year recent hospice experience.
3. At least two to three years’ experience in health care management, preferably in home
care operations.
4. Demonstrated ability to supervise and direct professional and administrative personnel.
5. Ability to deal tactfully with customers and the community.
6. Knowledge of corporate business management, governmental regulations, and private payer practices.
7. Demonstrates good communications, negotiation, and public relations skills.
8. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
