Job Summary:
The Case Manager for Substance Use Disorder (SUD) provides comprehensive support to individuals experiencing substance use issues, helping them navigate available treatment options, access resources, and develop strategies for long-term recovery. This role is central to coordinating care, advocating for clients, and collaborating with community resources to ensure that individuals receive effective, compassionate, and personalized support.
Key Responsibilities:
- Client Care Planning:
- Develop individualized care plans that address substance use treatment, mental health support, housing, employment, and other essential needs.
- Collaborate with clients to set achievable goals for recovery, ensuring alignment with clinical recommendations.
- Resource Coordination and Referral:
- Connect clients with appropriate treatment programs, including detoxification, inpatient and outpatient rehabilitation, counseling, and medication-assisted treatment.
- Coordinate with healthcare providers, mental health professionals, social services, and community organizations to streamline access to resources.
- Assist clients in obtaining benefits, such as Medicaid, housing assistance, and job training, to support stability in recovery.
- Ongoing Client Support:
- Maintain regular contact with clients to monitor progress, address challenges, and adjust care plans as needed.
- Provide education on substance use disorder, treatment options, relapse prevention, and coping strategies.
- Facilitate group and individual support sessions as appropriate, fostering a supportive recovery environment.
- Documentation and Reporting:
- Maintain accurate and confidential client records in compliance with legal and organizational standards.
- Document client interactions, progress, and treatment updates in case files and report to supervisors as required.
- Collaborate with team members and participate in case reviews to ensure quality care.
- Advocacy and Community Collaboration:
- Advocate for clients' needs within the healthcare, judicial, and social services systems.
- Develop and maintain relationships with community partners, including healthcare facilities, shelters, and addiction recovery centers, to enhance resource availability.
- Promote awareness of substance use disorder and destigmatize addiction within the community.
- Crisis Intervention:
- Respond promptly to clients in crisis situations, providing immediate support and referrals to emergency services when necessary.
- Coordinate crisis intervention services and provide follow-up to ensure clients are stable and supported.
Requirements:
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field
- Minimum of 2 years of experience in substance use disorder treatment, case management, or related field.
- Knowledge of substance use disorders, treatment modalities, mental health issues, and social support systems.
- Strong interpersonal, organizational, and crisis management skills.
- Ability to work with clients from diverse backgrounds in a non-judgmental, empathetic manner.
- Proficiency in documentation and case management software is preferred.
Work Environment:
- This position may require travel to meet clients in the community or accompany them to services.
