Position Title: Discharge Planner (Mental Health/Substance Use

Department: Crisis Stabilization Unit

Reports To: Clinical Manager / Supervisor

Location: Naples, Florida

Employment Type: Non Exempt

Schedule Options: Full Time (Guaranteed 40 hours, rotating weekends) PRN: Required to work 2 shifts per month, 1 of these shifts include a weekend shift

Job Summary:

The Discharge Planner for Mental Health and Substance Use plays a crucial role in coordinating post-treatment care and ensuring a smooth transition from inpatient or residential care to community-based support systems. The primary responsibility is to develop comprehensive discharge plans that address the mental health and substance use needs of clients, ensuring continuity of care and connection to community resources.

Key Responsibilities:

  • Assessment and Planning:

  • Conduct thorough assessments of patients' mental health, substance use, medical, and social needs to create individualized discharge plans.

  • Collaborate with the treatment team, including physicians, therapists, social workers, and nurses, to ensure a holistic understanding of the patient's needs.

  • Evaluate the client's readiness for discharge and address any barriers to a successful transition.

  • Coordination of Care:

  • Identify and arrange appropriate post-discharge resources, including outpatient therapy, substance use treatment programs, housing, support groups, vocational rehabilitation, and medical follow-up.

  • Coordinate with community resources, mental health agencies, addiction recovery programs, and other support services.

  • Develop a discharge plan that includes scheduling of follow-up appointments, transportation assistance, and ongoing communication with external providers.

  • Patient and Family Education:

  • Provide education and guidance to patients and families regarding the discharge plan, community resources, and strategies for managing mental health and substance use challenges.

  • Ensure patients and their families understand the importance of follow-up care and provide instructions for accessing services.

  • Documentation and Compliance:

  • Maintain detailed and accurate documentation of discharge planning activities, including assessments, care coordination efforts, and any referrals made.

  • Ensure all discharge planning follows federal, state, and local regulations, as well as organizational policies.

  • Monitor patient progress post-discharge and adjust plans as necessary, ensuring compliance with care protocols.

  • Collaboration with Multidisciplinary Teams:

  • Act as a liaison between patients, families, healthcare providers, and community resources.

  • Participate in multidisciplinary team meetings to discuss discharge options and contribute to the treatment planning process.

  • Crisis Intervention and Support:

  • Provide crisis intervention services as needed to support the immediate needs of patients during the discharge process.

  • Collaborate with crisis teams or emergency services when patients require immediate post-discharge support due to high-risk behaviors or unstable conditions.

Qualifications:

  • Education: Bachelor's or master's degree in social work, Counseling, Psychology, or a related field-Required

  • Experience:2 years of experience in mental health, substance use treatment, or discharge planning in a hospital or community setting

  • Familiarity with community resources for mental health and substance use treatment

  • Strong organizational and problem-solving abilities

  • Excellent communication and interpersonal skills

  • Knowledge of case management and discharge planning best practices

  • Ability to work independently and as part of a multidisciplinary team

  • Compassionate and non-judgmental approach to working with individuals dealing with mental health and substance use disorders

Working Conditions:

  • Work is performed in a residential setting

  • Some evening or weekend work may be required based on client needs/program needs

This position is key in supporting the long-term recovery of individuals with mental health and substance use disorders by ensuring they have the resources and support to continue their treatment and wellness after discharge.
