What are the responsibilities and job description for the SBHC Licensed Practical Nurse position at David Raines Community Health Center?
The SBHC Licensed Practical Nurse provides basic and skilled nursing care to the patients under the direction of the RN, NP or Physician. The SBHC LPN utilizes observational skills and reports pertinent patient symptoms and/or changes in conditions to the provider. The SBHC LPN maintain proper patient records and assist in education patients on medications, treatment, etc. The SBHC LPN may assist with emergency medical situations. The SBHC LPN assists with medical management duties such as, but not limited to, working up charts, maintaining adequate supplies and maintaining a clean organized, safe environment.
Essential Functions:
- Contributes to the assessment of patients and assures the well-being of patients by observing patients, charting, and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
- Administer medications and observe patients for adverse reactions to medications or treatments.
- Perform laboratory tests and therapeutic services.
- Educate patients/families about disease treatment plan including self care post discharge, holistic health needs, available resources and follow up care within scope of job, requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision.
- Must be able to work in a fast-paced environment and take appropriate action.
Minimum Qualifications:
Education:
- Graduate from an accredited school of practical nursing.
Experience:
- Minimum of two years experience as an LPN and demonstrated ability to perform the functions of the LPN according to accepted standards of nursing care.
- Experience with electronic health records required, preferably NextGen or similar EHS.
- Preferred FQHC or Healthcare experience.
License/Certification:
- License Licensed Practical/Vocational Nurse credentialed from the Louisiana Board of Nursing.
- BLS/CPR credentialed from the American Heart Associate (AHA). BLS/CPR card must be valid through employment.
Employment would be a full-time (40) hours per week/ non-exempt position during the SBHC calendar school year as established and approved by the governing school board. Work schedules are generally Monday thru Friday, based on clinical operations and during the approved SBHC calendar year (August - May). Work schedules are not a contract of employment and may change at the needs of the DRCHC.
