Job Summary
At David’s Loft Clinical Programs, we owe our success to people and processes. The Health Services Coordinator is a supportive force behind both, and we are currently seeking someone exceptional to take the helm. The ideal candidate will be responsible for providing administrative and clinical support to ensure the efficient operation of the Office of Health Services. The Health Service Coordinator works under the direction of the Director of Integrated Behavioral Health and provides oversight to medical care management for over 100 clients with co-occurring diagnoses of mental health and substance use disorders. The Health Services Coordinator is a LPN or RN and has 1-2 years of experience providing care management.
Responsibilities
- Ensures initial and follow-up medical appointments are scheduled to include:
- Primary Care;
- Specialist;
- Dental; and
- Vision.
- Serves as liaison between internal and external medical and behavioral health providers
- Ensures all patients/clients maintain physician orders
- Collaborate with program staff to ensure medical appointments are attended as scheduled
- Ensure program compliance with all applicable state, federal, and local laws, and regulations.
- Prioritize regular communication about the client’s needs with all relevant parties, including medical care managers, Medical Director, the prescribing practitioner, and external service providers.

- Coordinate transportation for medical appointments.
- Collect and maintain data in EHR.
- Ensure compliance for medicine storage cabinets for community housing and residential programs.
- Must have the ability to be on-call as needed to ensure the needs of clients are met, this includes weekends and holidays.
- Provide transportation to clients who require escorting to hospitals, dentists' appointments, checkups, and specialty appointments to ensure client advocacy is being delivered.
- Ability to develop, maintain, and implement department processes to ensure smooth operational integrity

Qualifications
- Medical administrative support experience
- Proficient in EHR systems such as Epic and Meditech
- Strong knowledge of HIPAA regulations and medical terminology
- Excellent patient service and clerical skills
- Familiarity with Microsoft Office and multi-line phone systems
- Maryland licensure as an LPN or RN
- Experience in medical office management and records handling
Job Type: Full-time
Pay: $68,000.00 - $80,000.00 per year
