In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease. This can include crisis counseling, mental health services, and connecting patients with community programs and resources.

  • Must be comfortable working between two clinic facilities - Marion County Dialysis and Franklin Dialysis.

Requirements of the position:

  • Master's degree in Social Work (MSW) required
  • Licensed or registered in the state of Indiana as required by state regulations.
  • Comfortable floating between two clinic facilities - mileage compensation available.
  • Demonstrated knowledge of government and private insurance programs
  • Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
  • Dedication to caring for patients suffering from chronic kidney failure.
