Job Summary
As the Intake Licensed Practical Nurse (LPN), you will be responsible for screening, processing and coordinating clinical referrals and services. Responsible for obtaining and maintaining updated information about the department, community, and State programs. Collaborate with homecare billing and insurance verification assistant for insurance verification and authorization for services at various timepoints such as admission, resumption of care and throughout the homecare episode.
Intake Licensed Practical Nurse (LPN) Job Responsibilities :
- Accepts and processes written, electronic, and telephone referrals for the Homecare and Hospice program. Ensures complete collection of clinical, financial and insurance data. Monitors patient status throughout the health care continuum - completes transfers to ensure accurate hand-off of information.
- Ensures verification of MD licenses of Physicians not on staff at DKH or UMASS by phone or via internet screens maintained by each States web site.
- Responsible for coordinating the admission process for new and returning home health and hospice patients.
- Under the supervision of the clinical manager, assesses eligibility for home health or hospice services based on initial referral information, including the patient’s medical needs, insurance reimbursement, and homebound status.
Determines if a Face-to-Face Encounter has been performed, collaborating with the patient’s provider to schedule a Face-to-Face Encounter within regulatory guidelines, preferably prior to admission to home health services.
Performs patient safety screens, to include search of judicial records, sex-offender registries, environmental screen, presence of weapons in the home, etc. and notifies clinical manager and assigned clinicians of any findings.
