Day Kimball Healthcare is hiring for a Clinical Supervisor Registered Nurse for the Full Time Days Shift 40-Hours HomeCare job in Putnam, CT.As the Clinical Supervisor / Patient Care Coordinator Registered Nurse, you will be responsible for serving as a communication hub for coordinating the multidisciplinary home health services provided by the clinical team. Providing vertical support between Homecare clinical staff and leadership, focusing these support efforts on fulfilling the agency's strategic goals and mission to provide high quality patient care. Supervising all home health aide staff and serves as clinical resource for scheduling. Ensuring the quality and progress of the HHA service in accordance with department objectives, standards and policies. - Actively participates in assuring quality care delivery by all field staff.
- Incorporates critical thinking and establishes priorities in decisions about patient care and staffing.
- Assists and provide input with the Clinical and Therapy Managers in the delivery of performance concerns, coaching and / or corrective actions.
- Maintains awareness of patients' needs and communicates changes to others.
- Works closely with Intake and Scheduling to ensure the delivery of services is timely and appropriate.
- Adjusts assignments and priorities in response to changing or increased workload.
- Oversees the delivery of patient care by the Home Health Aide and the scheduling of visits by the Clinical Team Assistant. Assures quality care and the utilization of services. Assists with daily management of HHA caseload. Monitors HHA visit notes for accuracy, completeness, verification and follow through.
- Is accountable for maintaining established productivity goals, continuity of care, and appropriate utilization of resources. Performs evaluations and supervises each HHA in the field in regard to quality of care / customer satisfaction. Participates in quality improvement by serving on a QI team or cross-functional team.
- Delegates responsibilities fairly according to each person's capabilities ensuring completion of all work.
- Acts as a patient and staff advocate.
- Practices cost control.
- Keeps Director and Clinical / Therapy Supervisors up to date on all pertinent matters.
- Supports nursing policies and acts as change agent when new procedures and policies need implementation.
- Serves as a resource and role model for all staff.
- Provides weekend / holiday coverage on a rotating basis as needed basis.
- Participates in staff development and orientation as needed.
- Participates in the process of interviewing, hiring and disciplinary action; completes performance reviews within established time frame. Participates in orientation and in-service education, completes competency evaluation of the HHA staff during orientation, makes competency visits twice annually and as needed to address HHA field competencies.Clinical Supervisor (RN) Education and Experience Requirements :
- Graduate from a Baccalaureate Degree program accredited by NLN, or Diploma or Associate Degree in Nursing required.
- Valid Connecticut driver's license and proof of current insurance coverage required.
- Current license to practice as a registered nurse in the State of Connecticut required.
