Our counselors are trained in a variety of evidence-based techniques and therapies to help our consumers achieve optimal mental health.
The Service Facilitator is responsible for coordinating recovery-based services for consumers who face mental health challenges. Service Facilitators develop assessments, recovery plans, make referrals for service and facilitate team meetings.
Responsibilities Include:
- Coordinate, monitor and manage care plans, services and resources regarding consumer caseloads.
- Coordinate the completion and compliance of required CCS consumer documentation.
- Ensure completion of CCS required certification, continuing education, and documentation of service facilitation and individual skill work.
- Complete and submit billing for all billable services and all other documentation within 48 hours.
- Coordinate with community agencies and other healthcare providers on the consumer’s team.
- Collaborate with interdisciplinary teams to optimize consumer outcomes.
- Provide a collaborative, productive workplace environment while enriching the team environment.
- Attend and actively participate in program meetings and agency meetings.
Position Qualifications:
- Master’s Degree in social work, psychology, or closely related field
- Demonstrated experience working with individuals or families on issues related to mental health, community resources usage, case management and/or advocacy.
- Commitment to quality care in strength-based, trauma-informed, recovery-focused programming
- Valid driver’s license; insurance and successful completion of a driver’s background check for work related travel.
- Demonstrated written and verbal communication skills with the ability to interact with consumers in a professional, empathetic, and confidential manner.
- Proficient computer skills, specifically in current versions of Microsoft Office Suite; and willingness to learn new skills as needed or required.
- Ability to pay attention to detail and a commitment to quality and the timely completion of duties and attainment of deadlines.
- Independent workers with the ability to create and maintain one’s own work schedule to meet consumer needs.
- Demonstrated ability to work as a member of a team.
