Summary
The Case Manager (Case Manager- MORES QP) provides a community-based service to the client and caregiver of a child/youth that has mental illness, substance use disorder or co-occurring diagnoses. Case Manager- MORES QP provides structured, one-to-one, person-centered, strength-based interventions that promote self-determination, self-advocacy and focus on recovery and resiliency. The service provided to the caregiver must be directed exclusively toward the benefit of the child/youth. A Case Manager- MORES QP is expected to improve the outcomes for child/youth that are involved in multiple systems and increase the child/youth and the familys capacity to manage their own services and support while promoting recovery and healthy relationships. The Case Manager- MORES QP service confirms the belief that family voice and choice are essential in care and in producing positive outcomes.
Essential Duties and Responsibilities:
Provide solution-focused behavioral health services that are responsive to child/youth and family, juvenile justice, social services, and school needs. Provide appropriate screening, early identification, and assessment of suicide risk, trauma exposure, substance use, exposure to and risk of violence, eating disorders, and other clinical presentations. Encourage the child/youth, family members and natural supports to drive all aspects of the planning and treatment process. Remain connected with the child/youth and family until they are firmly established and engaged (appointments scheduled, goals set, resources reviewed) with the services and supports to meet their needs through-out the follow-up period.
Participates in interdisciplinary treatment planning, consultation activities and ensures all involved parties are aware of the plan of care.
Provides crisis intervention to all participants of MORES and involves crisis services when needed.
Supervisory Responsibilities: None unless supervising a student and/or training a co-worker.
Education and/or Experience: Minimum of a Bachelors degree and experience that qualifies the staff person as a Qualified Professional (QP). Experience in mental/behavioral health/substance use crisis response, meeting the QP requirements, and at least 1 year experience working with child/youth and adolescents.
