Pay Scale: $17-$18hr
Summary:
Provides administrative support to the Tailored Care Management (TCM) Program by researching and verifying contact information for assigned TCM clients using all available resources. Ensures TCM engagement/consent can occur through a timely warm hand-off to TCM staff. Requires strict compliance with professional, legal and confidentiality standards. Position is temporary, could be part time or full time. Applicants must have phone, computer, and stable internet connection to perform required duties.
Essential Duties and Responsibilities:
- Researches and verifies contact information for clients assigned to agency for TCM using a variety of available online platforms (NCTracks, NCHealthConnex, Carequality, claims information, etc). Documents all work/findings in EHR following documentation guidelines. Coordinates warm hand-off to TCM staff to ensure TCM engagement/consent occurs timely.
- Ensures strict compliance with all professional standards including Medicaid and Medicare regulations, HIPAA, and agency policies and procedures.
- Other duties as assigned by supervisor to provide support to the TCM program.
