Facility: Children's Home Care of Dayton Department: Home Infusion Pharmacy Schedule: Full time Hours: 40 Job Details: The home infusion pharmacist is responsible for providing pharmaceutical care, primarily of an intravenous therapy nature, to home care patients. Consult and coordinate with physicians, nurses, third party payers, caregivers and patients in planning, providing, implementing, and monitoring effective drug therapy.
