Schedule: Monday through Friday, 4-10 hour shifts

Summary:
This is an exempt position responsible for providing Urology patient care under direct Physician supervision. Duties include but are not limited to assessing, diagnosing, prescribing, treating, and educating patients. This position works closely with physicians and other clinicians in a team approach to patient care in a clinical and hospital setting.

Essential Duties and Responsibilities:
- Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination;
- Determine tentative diagnosis and order appropriate laboratory and diagnostic tests;
- Consults with Physicians and other clinicians as an integral part of the clinical team to asses, evaluate, and address Urologic patient needs;
- Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws;
- Communicates with patient regarding medications, treatments, and discharge instructions as applicable.
- Educates patients regarding health promotion/illness prevention;
- Communicates appropriate case management information to members of the clinical team;
- Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner;
- Coordination of care for office patients. Field clinical phone calls and service as contact to patients and referring Physicians;
- Serves as first contact for after-hours patient calls as scheduled;
- Follow all health care privacy and safety rules and adheres to Dayton Physicians Core Values;
- Attendance and punctuality are essential requirements of this position to provide excellent customer service to both our internal and external stakeholders.

Supervisory Responsibilities:
This position does not contain supervisory responsibilities.
Required Education and/ or Experience:
- Graduate of an accredited advanced nurse practitioner curriculum
- Current state license as an advanced registered nurse practitioner with prescriptive authority
- Current CPR certification
- Current DEA license
General Skill Requirements:
- Excellent oral and written communication
- Working knowledge of Microsoft Office, including Word, Excel and Outlook.
- Experience working with a Practice Management system.
