What are the responsibilities and job description for the Family Mentor position at DCCCA?
Position Overview:
- Provides for the safety, stability and well-being of children and families during the referral period
- Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models
- Meets DCF timelines to connect with the family and facilitate the initial family meeting
- Engages and works in partnership with the case manager and family to complete assessments
- Responds to calls and client emergencies as needed including being on-call and working some weekends and public holidays as necessary
- Meets with clients at least once a week to offer one-one-one support
- Develops recovery plans, coping methods and rehabilitation services with the client
- Facilitates treatment assessments and referrals and provides client support in arranging for medical appointments, counseling, support groups and other resources as needed
- Monitors the client's behavior, schedule, medication and activities and reports any unusual behavior, conflicts or recovery deviations to the relevant professionals
- Provides transportation and resources and coordinates community support services as needed
- Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
Requirements include:
- A high school diploma or equivalent is required
- Must have lived experience with substance abuse, treatment and recovery with a minimum of three years of continuous recovery and be working with a stable recovery program (12-step, church/spiritual, other recovery support group)
- Requires knowledge of recovery resources and has experience interacting with individuals seeking recovery
- Personal experience with child welfare preferred
- Valid driver's license with a good driving record
- Must pass pre-employment background and reference checks, fingerprinting and drug screening
