- Responds promptly to crisis situations, documents critical incidents and is available, as needed, to clients
- Requires a minimum of 60 hours in home therapy per month
- Provides crisis/on call services and 24 hour, 7 days a week support to assigned clients
- Facilitates an initial meeting with the client and the referral source (as appropriate) within the required time frames
- Engages and works in partnership with the client to complete the initial, comprehensive assessment, including a mental status exam, resulting diagnosis based on the most recent Diagnostic and Statistical Manual of Mental Disorders
- Develops treatment plan goals and objectives, in conjunction with the client
- Provides individual, family and/or group counseling as appropriate to client need
- Functions as a client advocate and liaison with other community services
- Monitors client progress and service provision using the established tools, and updates treatment plans based on those assessments
- Maintains the client case record, documenting all client and collateral contacts in a timely manner in the electronic medical record
- Completes discharge summaries and aftercare contacts according to agency policy
- Provides written and verbal progress reports to referring entities within required time frames
- Works cooperatively with other program staff to ensure seamless, effective services
- Participates in regular clinical supervision
- Travel to home-based therapy may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day including early morning or evening hours
- Requires a Master's degree in human services or related field
- Requires 2 years of child welfare and/or family services experience
- Requires licensure by the Behavioral Sciences Regulatory Board at the master's level. Clinical licensure is preferred.
- Valid driver's license with a good driving record
- Must pass pre-employment background checks, reference checks, fingerprinting and drug screening
