Positions Available: 4 (2 Day Shift, 2 Overnight Shift)
Fern Park is currently seeking four compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team. We support individuals with intellectual disabilities in a home-like setting and are looking for professionals who are committed to providing high-quality care.
As an LPN at Fern Park, you will be responsible for delivering healthcare services in accordance with facility policies, the Nurse Practice Act, ICF/DD guidelines, and applicable state standards.
RESPONSIBILITIES:
- Support the development and ongoing evaluation of each individual’s Active Treatment Plan.
- Facilitate effective communication regarding individual health status and care interventions, both verbally and in writing.
- Manage and maintain accountability for scheduled and controlled substances in compliance with facility policies and pharmacy regulations.
- Ensure the medication rooms are secure and maintained to cleanliness standards.
- Monitor and maintain inventory of medications and nutritional (Enteral) supplies to ensure consistent care.
- Accurately transcribe physicians’ orders.
- Assist in personal care training programs, including hygiene, skincare, and self-medication.
- Participate in staff training, in-service sessions, and continuing education as needed.
- Attend all required trainings and demonstrate competency in learned skills.
- Complete assigned internal monitoring tools.
- Participate in investigations of unusual incidents as required.
HIPAA/PHI Access:
This position has access to protected health information (PHI) for the purpose of providing service and supports to individuals in the facility and has authorization to disclose PHI as outlined in company policy and procedure. Any unauthorized use or disclosure of PHI will result in disciplinary action, up to, and including termination of employment.
