About the Job:
- You will be responsible for assessing the physical, psychosocial, and environmental needs of our patients and their families.
- Develop and implement individualized plans of care that address the unique needs of each patient.
- Maintain accurate and timely documentation of patient progress and outcomes.
- Collaborate with the patient/family, attending physician, and other members of the IDG to ensure comprehensive care.
Key Qualifications:
- One year of clinical experience as an RN.
- Current RN licensure in the state of practice and one year of clinical experience.
- Strong communication and interpersonal skills.
