What are the responsibilities and job description for the Hospice Nurse Coordinator position at Deaconess Hospice?
We are seeking a compassionate and skilled RN Case Manager to join our Hospice team at Deaconess Hospice in Brookhaven, MS. Responsibilities:
- Makes initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identifies patients' physical, psychosocial, and environmental needs, reassessing as needed, no less than every 15 days.
- Initiates and coordinates patient care plans, maintaining accurate clinical notes.
- Collaborates with patients, families, attending physicians, and IDG members to provide daily care and support.
Patient Care Responsibilities:
- Instructs and supervises patients and families in self-care techniques when appropriate.
- Maintains accurate and relevant clinical notes regarding patients' conditions.
- Reports patient conditions and identified needs/issues to supervisors daily.
Requirements:
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
