Job Summary
We are currently seeking a skilled RN Case Manager to join our multidisciplinary team. The ideal candidate will possess a deep understanding of hospice care principles and practices, along with excellent communication and interpersonal skills.
Responsibilities:
- Makes initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identifies patients’/families’ physical, psychosocial, and environmental needs, reassessing them every 15 days or as needed.
- Develops and coordinates patient care plans, documenting relevant information and collaborating with other healthcare professionals.
- Supervises home health aides/homemakers, ensuring they meet performance standards.
Requirements:
- Current RN licensure in the state of practice.
- One year of clinical experience in hospice care or a related field.
- CPR certification and valid driver's license.
