What are the responsibilities and job description for the Life Care Manager/Guardian position at Decades/Aging Life Care Management?
Who we are: A locally owned community based care management and medical fiduciary company that is made up of RN and Social Work Care Managers working with older and disabled adults. Our work includes conducting assessments, resource coordination, consultation with seniors and families, medical advocacy; all to enhance the quality of life of those we serve. When court appointed or through Healthcare POA's, we work as a team to make medical decisions for those we serve. Creativity, collaboration with community partners and professional experience are critical to the success of our work.
Who we are looking for: Are you interested in a new, and growing area of practice. Working in institutional systems can be very limiting in terms of your work with clients and patients. Our new Geriatric Care Manager/Guardian Coordinator will be an individual seeking work that allows them to follow the life of clients and families through the aging process. An understanding of resources and systems allows you to guide those you work with through the confusing maze of aging options. You are uniquely qualified to advocate for your clients when medical matters are involved. Who is that person: We require 2-3 years of Care/Case Management experience with disabled adults and/or seniors. Bachelor's/Masters in Social Work or Nursing preferred. Experience in Hospice, Home Care, Hospital, Case Management or Community Care Management preferred. You must be able to work independently as well as with a team, and be flexible to work with cognitively impaired adults, their families, various professionals. Mental Health experience a plus.

Schedule: Monday to Friday (some flexibility here). Week-end/Evening On-Call Rotation every six weeks.
