About the Role:
The Discharge Planner plays a critical role in ensuring a smooth transition for patients from inpatient care to their next phase of recovery, whether that be home, rehabilitation, or long-term care. This position involves comprehensive assessments of patients' needs, developing individualized discharge plans, and coordinating with various healthcare providers and community resources. The Discharge Planner will work closely with patients and their families to educate them about post-discharge care and available resources, ensuring they feel supported and informed. Additionally, this role requires ongoing communication with medical staff to address any potential barriers to discharge and to facilitate timely and effective care transitions. Ultimately, the Discharge Planner aims to enhance patient outcomes and satisfaction by providing a structured and supportive discharge process.
Responsibilities:
- Conduct thorough assessments of patients' medical, emotional, and social needs prior to discharge.
- Develop and implement individualized discharge plans in collaboration with healthcare teams and community resources.
- Educate patients and their families about post-discharge care, available resources, and follow-up appointments.
- Coordinate with various healthcare providers, including social services and long-term care facilities, to ensure continuity of care.
- Monitor and evaluate the effectiveness of discharge plans, making adjustments as necessary to meet patients' evolving needs.
Minimum Qualifications:
- Diploma, Degree, or certification in field or related field (SUD, Mental Health, Social Work, Medical...)
- Experience in case management or discharge planning within a healthcare setting.
- Knowledge of Medicaid and other insurance processes related to discharge planning.
