Residential Social Services Worker
Monday- Friday 8am-4pm
The Residential Social Services Worker is an integral part the care team that helps change the lives of our staff, and residents each day. You will work to evaluate and ensure the care of our residents. The Residential Social Services Worker works with our Personal Care and Independent Living Residents on a day to day basis.
In this role you will :
- Support the Social Services Director with department planning and organization necessary to ensure patient / resident medically related, emotional and social needs are maintained on an individual basis
- Develop and maintain good working rapport with patients / residents, family members, interdisciplinary team members, service providers and facility staff to ensure that social services programs can be properly maintained to meet the needs of patients / residents
- Participate in the admission and discharge planning processes, conducts patient / resident assessments, care planning process and the coordination of resident room changes. Ensures patient / resident names are on the wall plaque of their room
- Enter assessments, support plans, notes, etc. into PointClickCare and / or in department logs, Excel database, etc. Assists in maintaining department reports and records are survey ready
- Investigate residents’ complaints regarding rooms, lost items, etc. provides information to Residential Administrator and assists in the resolution of complaints
- Other duties as assigned
