OVERVIEW:
The Social Services Coordinator will work to enhance the quality of life of senior housing residents. This is accomplished by determining service needs, providing access to the entire service system, ensuring effective and coordinated delivery of service to each individual client and facilitating senior participation in programs that enhance their physical, social and mental well-being. Service coordination is provided to residents of senior housing owned, operated and/or managed by the NewCourtland.

REQUIREMENTS:

Education: A bachelor’s degree, preferably in social work or a related field is preferred.
Work Experience: One year of experience in the aging or human service system is preferred.
A combination of education and experience that results in knowledge of the human service system and experience working with older adults and persons with disabilities is essential.

RESPONSIBILITIES:

Provides general assistance and advocacy related to social and supportive services to all residents; provides up-to-date information and clarification regarding programs such as Medicare, Medicaid, entitlements as well as other supportive and social services.

Conducts outreach to new residents to provide a welcoming environment, inform of available services and help ensure a smooth transition to a new home.

Provides comprehensive assessments and periodic reassessments of all residents who may need assistance; assessments may lead to the development of a care management plan.

Provides counseling or other assistance to residents and their families for problem resolution that arises as part of the service coordination process.

Develops and maintains linkages with community resources such as the Area Agency on Aging in order to remain current regarding information and services available to address resident needs.

Determines appropriate intervention for crisis situations, resident conflict resolution and critical situations negatively impacting residents, i.e., Protective Services, abuse, mental health deterioration and family conflicts.

Identifies gaps in available service provision. Researches and develops innovative, viable options to meet resident needs.

Acts as a liaison between residents and building management; manages the balance between advocacy for the resident and needs of building management.

Covid Vaccination is a requirement for employment at NewCourtland. If you are not currently vaccinated, you will be required to receive the vaccination prior to hire date if you are offered employment unless you request and receive an approved medical or religious exemption from NewCourtland.
