We are seeking a Clinical Care Coordinator to join our team and support the behavioral health needs of military personnel and their families.

Responsibilities
As a Social Worker (Substance Abuse), you will conduct ongoing behavioral health risk assessments, determine health intervention needs, and monitor behavioral health treatment.
You will also collaborate with the Soldier Recovery Unit, Soldier and Family Assistance Center, and designated Veterans Benefits Administration staff to develop and facilitate individual Comprehensive Recovery Plans for each Soldier.
Requirements
To qualify for this position, you must meet the following requirements:
- Master's degree in Social Work from a Council on Social Work Education-accredited program.
- Current, active clinical license to practice social work independently.
- Minimum one year of specialized experience conducting behavioral health risk assessments and developing treatment plans.
