What are the responsibilities and job description for the Public Health Nurse (Nutritionist) position at DeKalb County Health Department?
The DeKalb County Health Department is seeking a part-time (22.5 hours per week) Public Health Nurse (Nutritionist). The Public Health Nurse (Nutritionist) functions as a member of the Women, Infant and Children (WIC)/Family Case Management (FCM) Team under the direction of the WIC/FCM Program Coordinator.
Responsibilities
- Performs nutrition services including nutrition assessments, determines nutritional risk factors, provides evidence-based nutrition counseling, goal setting, nutrition education, and assigns appropriate tailored food package benefits to prenatal, postpartum, breastfeeding women, infants, and children from birth to five years of age.
- Completes dietary assessment; assesses participant’s nutritional status: develops and coordinates an individualized nutrition plan of care, including involving the participant/family in the development of the participant’s goals and collaboration with other health care professionals as necessary.
- Implements the nutrition interventions identified in the nutrition plan of care including: participant-centered counseling on nutrition and dietary recommendations/prescription, evaluating effectiveness and outcomes of care, and revising the nutrition plan of care as needed.
- Determines WIC Program eligibility for WIC applicants per federal and state guidelines.
- Assesses the need for and makes appropriate medical and community social service referrals
- Assigns WIC eligible participant’s food package, issues WIC food benefits and explains use.
- Provides breastfeeding counseling and support.
Requirements
- Licensed RN with a strong background in maternal and child nutrition, lactation, nutrition assessment and nutrition education.
- One year nutrition/maternal child health (or related) experience in health care or community services setting preferred.
- Requires excellent verbal and written communication skills and the ability to quickly establish and maintain positive relationships with the public and employees.
- Ability to demonstrate proficiency in work-related tasks upon completion of formal and informal training.
- Bilingual (English/Spanish or English/Arabic) preferred.
