This position is responsible for answering questions pertaining to documentation, chart abstraction and review of medical records/clinical information for appropriateness, congruency and accuracy as it relates to physician orders. Participates in continuous quality improvement program audits to promote optimal staff/Patient outcomes.

Requirement:
- Medical Assistant (MA) or a qualified clerical professional with relevant experience
- Knowledge of medical terminology and hospice medical record
- Hospice related experience and/or training experience preferred with understanding of hospice policies and procedures
- Excellent analytical skills and attention to detail
- Excellent communication skills for effective team collaboration, along with strong problem-solving and critical-thinking capabilities.
- Good independent judgment Hospice experience preferred Proficient in accessing/updating information needed using Excel/dropbox.

Qualifications:
- ICD10 coding/OASIS experience a huge plus but not necessary
- Strong knowledge of medical terminology, basic anatomy and physiology
- Able to read and interpret physician/nursing medical records
- Willingness to undergo an intensive training period
- High attention to detail, organized, diligent and the ability to multi-task
- Analytical mindset, with great attention to detail
- Solid verbal and written communication skills
- Must be able to type 30-45 WPM or more
- Self-motivated individual capable of efficient time management
- Critical thinker capable of working collaboratively with the team
- Must be very reliable.
- Must be able to deal with pressure resulting from responsibilities.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Ability to interpret an extensive variety of instructions in various forms and deals with several abstract and concrete variables.
- Experience with Kinnser is preferred
- Previous experience in quality assurance or clinical documentation preferred.
Job Responsibilities:
- Documentation Review: Evaluate patient care documentation for consistency and accuracy with the Comprehensive Assessment and Plan of Care. Ensure the discipline frequency aligns with assessment data.
- Quality of Care Review: Monitor charts for quality of care rendered, including orders followed and appropriate contact with physicians and other disciplines. Resolve any charting issues promptly.
- Monitor and evaluate safety, risk management, infection control, and competency programs.
- Works collaboratively with clinical managers to assure documentation is complete, accurate, reflects care delivery and legal requirements.
- Performs Agency's clinical record reviews.
- Performs job in compliance with Agency policies and procedures and professional and community standards. Provides services in accordance with applicable state, federal, CHAP, and JCAHO standards.
