Role Description
This is a full-time on-site role as a Home Health Care Coordinator at Delano Home Health Care Inc. in Los Angeles, CA. As a Home Health Care Coordinator, you will be responsible for coordinating home health care services for patients. Your day-to-day tasks will include assessing patient needs, developing care plans, scheduling and coordinating home visits, communicating with healthcare providers and families, monitoring the quality of care, and ensuring regulatory compliance.

- Ability to assess patient needs and develop care plans
