Job Description :
Build trusting relationships with patients and their families and utilize dietary education and planning to directly affect their quality of life.Complete patient assessments and create patient-specific nutritional plans.Provide ongoing nutritional support.Comply with all Company policies and procedures, as well as federal, state, OSHA, and other agency regulations.Handle patient information with discretion and maintain HIPAA compliance.Utilize tactful and professional communication to promote positive relations with patients, families, physicians, co-workers, agencies, and service providers.Ensure patients receive appropriate care related to renal diet and medications.Provide comprehensive nutritional assessments for dialysis patients, ongoing instruction, monitor adherence to diet, provide educational material, and review monthly lab work.Assist with obtaining nutritional resources and protocols.Participate in the completion of Comprehensive Assessments and Plans of Care for all patients as a member of the Interdisciplinary Team.Participate in the Quality Assessment Performance Improvement process, monitor clinical indicators, attend meetings, and assist with action plans.Stay updated with changes in renal nutritional services.Review monthly lab work and make dietary and medication changes in conjunction with physicians.Assist in managing medications and obtaining nutritional supplements for patients.Refer patients for assistance with nutritional resources such as financial assistance, community resources, or in-home assistance.Perform job duties in accordance with all state / licensing requirements.Assist with other projects and assignments as requested by Clinic Leadership.Coordinate with other service providers as needed.Participate in national and state programs on renal nutritional services.Client Details
