What are the responsibilities and job description for the HIV Case Manager position at Delaware HIV Services?
The Delaware HIV Consortium (DHC) works to improve the lives of people living with and vulnerable to HIV through leadership, services, and advocacy. Founded in 1991, DHC has a long history of serving as the State's primary administrator of supportive services for people living with- and vulnerable to HIV. By working collaboratively with community partners and by providing direct services itself, DHC ensures that Delawareans living with HIV/AIDS have access to HIV case management, transportation assistance, a housing assistance program, emergency financial assistance, HIV community planning, outreach, education, testing and linkage to care, and advocacy. Additionally, the Consortium administers programs such as a HIV prevention education campaign, a data to care re-engagement program, a mail order condom distribution program, and a PrEP Navigation program. DHC provides services statewide through offices in Wilmington and Milford, Delaware.
Position Overview:
This position provides direct client services in the delivery of case management services to persons living with HIV.
Essential Job Function:
_ Essential Duties:_
- Conduct bio-psychosocial assessment of each client receiving case management services
- Develop an individual service care plan for each client (ongoing)
- Determine client eligibility for Ryan White services and a broad range of auxiliary services
- Assist clients in applying for services and benefits
- Coordinate, monitor, and follow-up on needed client services
- Conduct home visits, medical visits, and other community visits with clients
- Provide treatment adherence assistance
- Ensure service delivery aligns with policy, regulation, and case management standards
- Ensure service data and case notes are entered within 2 business days
- Ensure client hard charts are in good order for state audits
- Ensure monthly reporting is generated and submitted on time
- Attend and participate in the state DPH Case Management required trainings
- Assist eligible and appropriate clients who may benefit from enrolling into the 340B program
- Develop individual professional development goals annually
- Other duties as assigned
Qualifications:
MINIMUM EDUCATION AND EXPERIENCE
- Master’s or a Bachelor’s degree in social work, psychology, public health, or related field one (1) years of qualifying experience.
- Qualifying experience means verifiable full-time employment in a human services or related field.
- Non-profit experience preferred.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES
- Excellent verbal, written and organizational skills, proficiency with Windows and Microsoft Office
- Strong understanding and knowledge of CAREWare
- Attention to detail
- Good time management skills are necessary.

Working Conditions:
- The HIV Case Management program is a client centered program, and as such, requires travel for home visits and medical visits, as well as occasional training.
- The person holding this position must hold a valid driver’s license and have a reliable vehicle.
- Mileage reimbursement is offered for appropriate work related travel.

Pay: $36,000.00 - $39,000.00 per year
